For instance, we may be interested in determining if a new diagnostic test is noninferior to the standard reference test because the new test might be inexpensive to the extent that some small inferior margin in sensitivity or specificity may be acceptable. Noninferiority trials are also found to be useful in clinical trials, such as image studies, where the data are collected in pairs. Conventional noninferiority trials for paired binary data are designed with a fixed sample size and no interim analysis is allowed. Adaptive design which allows for interim modifications of the trial becomes very popular in recent years and are widely used in clinical trials because of its efficiency. However, to our knowledge there is no adaptive design method available for noninferiority trial with paired binary data. In this study, we developed an adaptive design method for non-inferiority trials with paired binary data, which can also be used for superiority trials when the noninferiority margin is set to zero. We included a trial example and provided the SAS program for the design simulations.

Adaptive Group Sequential Confidence Intervals for the Ratio of Normal Means

In controlled group sequential trials, we consider the ratio of normal means as the effect measure of interest and derive group sequential nested confidence intervals on this parameter. In an interim analysis, the sample sizes of the following stages can be determined in a completely adaptive way using the unblinded data from all previously performed stages. Despite the data-dependent adaptatio...

Gefitinib for recurrent non-small-cell lung cancer: all things are not created equal.

Two studies reported in this issue of Journal of Clinical Oncology have results in which the distributions of outcomes for patients with non–small-cell lung cancer treated with gefitinib were similar to those for standard chemotherapy treatment control arms using docetaxel and vinorelbine. The important question is to what extent these two studies provide evidence that the efficacy of gefitinib...

Equivalence and noninferiority testing in regression models and repeated-measures designs.

Equivalence and noninferiority designs are useful when the superiority of one intervention over another is neither expected nor required. Equivalence trials test whether a difference between groups falls within a prespecified equivalence region, whereas noninferiority trials test whether a preferred intervention is either better or at least not worse than the comparator, with worse being define...

Evaluating Intermittent Androgen-Deprivation Therapy Phase III Clinical Trials: The Devil Is in the Details.

PURPOSE Intermittent androgen deprivation (IAD) has been widely tested in prostate cancer. However, phase III trials testing continuous androgen deprivation (CAD) versus IAD have reached inconclusive and seemingly contradictory results. Different design and conduct issues must be critically evaluated to better interpret the results. PATIENTS AND METHODS Seven published phase III trials were e...

Understanding noninferiority trials

Noninferiority trials test whether a new experimental treatment is not unacceptably less efficacious than an active control treatment already in use. With continuous improvements in health technologies, standard care, and clinical outcomes, the incremental benefits of newly developed treatments may be only marginal over existing treatments. Sometimes assigning patients to a placebo is unethical...
